Enrollment
Upon admission to Dubuque Montessori School, the following forms must be completed and submitted in accordance with the Department of Human Services Licensing Standards before a child may begin.

- Travel, Activity & Media Release Form
- Permission to Apply Sunscreen
- Child and Adult Food Care Program Application and Enrollment Form
- Signed Statement Acknowledging Review of the Dubuque Montessori Parent Handbook
